2. Download and complete the Registration Form (pdf) return it to the address on the form with full payment.
   
   
3. Call Registration Services at (435) 797-0423 or 1(800) 538-2663. They will register you with a Visa, MasterCard, Diners, Discover or AMEX Card Number.
   
   
4. Fax your registration form with your credit card number. The 24-hour fax number is (435) 797-0636.

Note: Printed conference Proceedings are included with all conference registration fees. Conference Proceedings are available for $45 otherwise. All registration items will increase by $20 after March 31. No refunds will be given on conference fees after April 11, 2010 and the Wednesday banquet and the Thursday tour are non-refundable.

Note: Vendor registration includes one table, one copy of the printed Proceedings, and entitles one company representative to attend the conference presentations. The vendor’s spouse is welcome to participate in all WSC sponsored spousal events. Additional vendor representatives are expected to register separately as regular conference attendees. The Wednesday banquet and the Thursday tour are non-refundable. No conference fees will be refunded after April 11, 2010.
