Early-bird rates are offered through October 1, 2012. Group Registrations: Groups must complete an individual form for each attendee. If paying by PO, simply use the same PO number on each form. Cancellation/Refund Policy: Refunds will be made to those registrants who must cancel, less a $25 processing fee. Written cancellation must be received by October 10, 2012. No refunds will be made after that date. Substitutions are always accepted. Utah State University reserves the right to cancel this program due to insufficient enrollment and limits its liability to registration refunds only. Registration Fees are in US Dollars.
